-0165(2)(d) - A plastic bag was observed covering the top of the high chair back for improved cleaning and sanitation purposes per provider statement. -0165(2)(c) - The laptop with cords was observed behind the couch on the floor, within reach of children. (The provider removed the lap-top and cords during the visit) -0165(3)(a) - A few boards with protruding nails were observed along the fence to the right of the house. -0165(3)(g) - The high chair observed in the kitchen has a cracked, flaky, and damaged surface. -0165(4)(e) -The water temperature was measured at 126.5 degrees Fahrenheit.
